# Modified Colorado Symptom Index (MCSI)

> MCSI response scale codes for Ascent surveys

**Field format:** `9(2)` -- two-digit numeric string **Valid values:** `00`-`04`, `97`, `98`

The MCSI is a 14-item self-report instrument. Each item uses the same response scale:

| Code | Response |
| - | - |
| `00` | Not at all |
| `01` | Once during the month |
| `02` | Several times during the month |
| `03` | Several times a week |
| `04` | At least every day |
| `97` | No response / Don't know |
| `98` | Declined to answer |
