# Stage of Change

> Stage of change codes for ACT and Ascent SUD surveys

**Field format:** `9(2)` -- two-digit numeric string
**Valid values:** `01`-`06`

For individuals with a SUD diagnosis.

| Code | Stage |
| - | - |
| `01` | Pre-contemplation |
| `02` | Contemplation |
| `03` | Preparation |
| `04` | Action |
| `05` | Maintenance |
| `06` | Relapse |
