# End of Treatment Reason

> Reason treatment was not completed codes for CoBHRM

**Field format:** `9(2)` -- two-digit numeric string
**Valid values:** `01`-`11`

| Code | Reason |
| - | - |
| `01` | Attendance |
| `02` | Client Decision |
| `03` | Client stopped coming and contact efforts failed |
| `04` | Financial/Payments |
| `05` | Lack of Progress |
| `06` | Medical Reasons |
| `07` | Military Deployment |
| `08` | Moved |
| `09` | Incarcerated |
| `10` | Died |
| `11` | Agency closed/No longer in business |
