# Family Substance Use Disorder

> Family SUD relationship codes for Special Connections

**Field format:** `9(2)` -- two-digit numeric string
**Valid values:** `00`-`05`

Used for both historical and current family substance use disorder.

| `01` | Mother |
| `02` | Father |
| `03` | Siblings |
| `04` | Extended Family |
| `05` | Spouse/Partner |
