# ACT - Assertive Community Treatment

## Overview

The ACT (Assertive Community Treatment) schema captures data for intensive community-based mental health treatment programs. ACT teams provide comprehensive services to individuals with severe mental illness who require intensive support to live successfully in the community.

> This schema is **only required** for clients enrolled in ACT programs (where `ACT_ENROLLMENT = 1` on the Admission record). Submit ACT records for each reporting period during enrollment.

### Minimal Valid Example

Quarterly update for an ACT-enrolled client with no psychiatric or incarceration days.

ACT discharge with outcome and comments. Discharge date must be within the same fiscal year quarter as the assessment date.
