# ASA, ASQ, ASD - ASCENT

> Ascent Colorado is a community-based program that provides treatment and support to youth and young adults 15-29 years old who may be experiencing an early onset of psychosis.

## Overview

The ASCENT schema captures data for the First Episode Psychosis (FEP) program. It consists of three record types:

* **ASA** - Ascent Admission: Initial assessment at program entry
* **ASQ** - Ascent Quarterly Update: Quarterly progress assessments
* **ASD** - Ascent Discharge: Final assessment at program completion

> This schema is **only required** for clients enrolled in Ascent/FEP programs (where `ASCENT_ENROLLMENT = 1` on the Admission record). If a client has a previous discharge, they must open a new Admission (ASA) before proceeding to quarterly updates (ASQ).

Initial Ascent (FEP) admission. Includes `REFERRAL_SOURCE` and `PRIOR_SUICIDE_ATTEMPTS` which are required only at admission. Client must be at least 13 years old.
