01 | Full Engagement — Individual is participating fully in all requested activities based on their choosing |
02 | Moderate Engagement — Individual is participating in a fair number of requested activities based on their choosing |
03 | Low Engagement — Individual is participating in a low number of requested activities based on their choosing |
04 | No Engagement — Individual has expressed willingness to participate in services (at intake or via assertive outreach), with no engagement in Ascent activities |
05 | Refusal — Individual actively refuses offered services |
06 | Unable to Engage — Individual is unable to engage in services due to factors outside of their control. These could include being hospitalized/inpatient for most of the quarter, being incarcerated, or unhoused without a consistent means of communication |
